Mammography is considered as the main technique for thediagnosis of breast cancer, but its sensitivity and specificity are low in dense breasts. Therefore, several modalities of breast imaging have been used as adjuvant in cancer screening in women with dense breasts, one of them being breast tomosynthesis. This study aims to understand the contribution of breast tomosynthesis in the diagnosis of cancer in fibroglandular breasts. This is an integrative review through searches of the main journal databases, such as Scopus, SciElo, PubMed and Lilacs, using the following criteria: time frame of the last six years until the date of the research (2016 to August 14, 2021); search terms "digital mammography", "breast tomosynthesis", "breast cancer" and "fibroglandular breast" in English, present in the title; full text available in electronic format, in English, Portuguese and Spanish. Twenty-one articles were selected for reading in full and analysis. The bibliometric analysis showed that the largest number of publications occurred in the year 2019, the author who published the most was Nehmat Houssami and the country that published the most on the topic was the United States of America. Parameters with respect to tomosynthesis in comparison and aid to mammography were evaluated, such as cancer detection rate, false positives and negatives, true positives and negatives, sensitivity, specificity, diagnostic accuracy/accuracy, characterization, benign and malignant findings, positive predictive value (PPV), recall rate, biopsy rate, and interval and additional cancers. The addition of tomosynthesis to mammography brought advantages to dense breasts, showing superior benefits over mammography in this population. It was possible to observe through this study that tomosynthesis can be used in dense breasts as a primary technology, associated with another imaging method.
